24 / 7 Pet Return Program

1. 24/7 Pet return will for our "platinum level members" provide, for one year from todays date, for the 24 hour pick up and return of the pet(s), should they become lost and the owner cannot be contacted, provided they are found within a 50 mile radius of the veterinary practice (selected on my record) who has issued the membership to my pet. Under no circumstances will 24/7 Pet Return be required to pick up a pet that is found outside this 50-mile radius. 24/7 pet Return will however make every effort to continue to contact the pets owner and the veterinary practice who issued this membership to let them know the pets whereabouts if the pet is found outside a 50-mile radius. I the client understand that it is my responsibility to make sure that If I have a platinum level membership that it is indicated on my record at 247petreturn.com. If my membership is indicated as gold but should be indicated as platinum it is my responsiblity to notify my veterinarian or 24/7 Pet Return of the error.

2. 24/7 Pet Return will only pick up any one pet(with a platinum level membership) three times within 12 months from todays date. If an animal is picked up three times within the next 12 months, 24/7 Pet Return will require a deposit from the animals owner of $50.00 upon delivery of the animal after the third pick up. Your pet will be delivered to you when we pick them up for the third time in a 12 month period but we cannot pick your pet up for a fourth time unless the $50 deposit is paid at that time for the fourth pick up. This is not a charge for the third pick up it is pre-payment for a fourth pick up should it occur. If the animal is then picked up for a fourth time by 24/7 Pet Return the $50.00 will be used to pay for that pick up and another $50.00 deposit will be required for the next pick up. If the animal is not picked up and the deposit is not used then the deposit will be refunded in full. If you (the owner) elect not to pay the $50.00 deposit after the third pick up then the animal will not be picked up for a fourth time. 24/7 Pet Return will however continue to make its best effort to notify you and your Veterinarian as to the animals whereabouts.

3. I agree that if my pet is in need of emergency medical attention that I give 24/7 Pet Return permission to have my animal treated in such a way that they will be in no life threatening danger until I can be contacted. I further understand that 24/7 Pet Return will make every effort to return the pet to the practice, indicated on my record, for treatment but 24/7 Pet Return may at its sole discretion take my pet to any Emergency Pet Clinic or Veterinarian if 24/7 Pet Return deems necessary. I further understand that 24/7 Pet Return will pay all charges (authorized by 24/7 Pet Return) incurred at an Emergency Veterinary Clinic involved in the stabilization of my pet in the event I can't be contacted and my pet is in a life threatening situation. Under no circumstances will 24/7 Pet Return be responsible for any charges related to the treatment of my pet that have not been preauthorized in person, on the same date that the emergency treatment is administered, by an authorized 24/7 Pet Return Agent. Stabilization is defined as getting my pet to a state where a reasonable veterinarian beleives that the animal is not in danger of losing their life. 24/7 Pet Return is never responsible for any treatment administered to any pet unless a 24/7 Pet Return agent delivers the pet to the Veterinarian providing the treatment and the 24/7 Pet Return agent has authorized the treatment in person prior to treatment being given.

4. I agree to hold this Veterinary practice and 24/7 Pet Return harmless for any injury to or for the death of my pet, while in the possession of 24/7 Pet Return or any animal emergency clinic or veterinarian that my pet is taken to by 24/7 Pet Return.

5. I understand that 24/7 Pet Return, upon picking up my pet, will keep the pet in a safe environment until such time as the practice indicated on your pet's record opens for business at which time the pet will be returned to that practice and kept until I can be contacted. 24/7 Pet Return will charge no boarding fees for the time while the pet is being taken care of or boarded by 24/7 Pet Return. I understand that once my pet is transferred to my Veterinarian that I will be responsible for boarding fees charged by my Veterinarian and fees for any treatment my Veterinarian deems necessary for my pet until the time when I can be contacted and come to pick up my pet.

6. I understand that if I (the owner) cannot be located (for whatever reason) that 24/7 Pet Return will safely keep my pet for a period of forty five (45) days. At the end of forty five (45) days, if I cannot be contacted, the pet will be designated as abandoned by 24/7 Pet Return. 24/7 Pet Return will continue to try and contact me to let me know of the pets location for an additional 30 days. At the end of this period I will be deemed as having abandoned the pet and 24/7 Pet Return will have the right to find the pet a new home by whatever means 24/7 Pet Return deems appropriate.

7. I understand that 24/7 Pet Return has no control over the party that finds my pet. 24/7 Pet Return can only make suggestions as to what the finder should do when they find my pet or if my pet needs medical attention. 24/7 Pet Return is not an ambulance service for pets, however, we will make every effort to get your pet qualified care as quickly as possible but we will not be able to provide emergency medical services for your pet. I further understand that 24/7 Pet Return cannot be called upon to look for my lost pet. 24/7 Pet Return can only pick up your pet if they have a platinum level membership and have been found within a 50-mile radius of the Veterinary office where you obtained the pet's membership and the person finding the pet contacts our 24-hour number. I understand that it is my responsibility to make sure the 24/7 Pet Return 24-hour number is correctly stamped on my pets tag. I further understand that it is may sole responsibility to make sure my pet's membership expiration date on my pets record at www.247petreturn.com is correct. I understand my veterinarian is responsible for entering the correct membership expiration date on each of my my pet's records at www.247petreturn.com upon my intial purchase of my pets membership as well as any subsequent renewals of my pet's membership. 24/7 Pet Return cannot be held responisible for not providing assistance to pet's with incorrect expiration dates due to an entry error by the veterinarian or a member of their staff. I understand it is my responsiblity to log into my account and make sure my pet(s)membership expiration date is correct.

8. I understand that I do have the right to cancel my account at anytime. I do further understand that cancellation of my account will cease any and all obligation to your pet by 24/7 Pet Return. I further understand that I am entitled to no refund in whole or in part for any fees paid to my Veterinarian for my pet(s) membership if I elect to cancel. 24/7 Pet Return cannot provide assistance to any pet with a cancelled membership.

I have read and fully understand this agreement for the Pet Return program provided by 24/7 Pet Return and I acknowledge that I have been made no promises other than those expressly stated above. I have been made no oral promises by my Veterinarian or 24/7 Pet Return.
